What is Avow Care Services?

Avow Care Services provides a variety of services to
help with illness management. Our services are aimed at
providing symptom relief. With a layered approach, our
goal is to assist patients with addressing uncomfortable
conditions that negatively impact quality of life.

Avow was founded in 1983 as Collier County’s original,
nonprofit hospice. Today, Avow’s nonprofit companies
provide a variety of services for people facing chronic or
serious illness and hospice care and bereavement support
services for children and adults.

Remote Patient Monitoring
Remote patient monitoring uses technology that allows
patients to be monitored outside of a clinical setting, using
digital devices to collect and provide health information
to their healthcare provider from the comfort of their
home. It can help manage chronic conditions and serious
illnesses and can improve access to care and reduce the
unnecessary healthcare visits that can occur with a serious
illness. This service uses cellular-based technology to
monitor physiological metric such as:
blood pressure, blood sugar, oxygen saturation,
temperature and weight.
Principal Care Management
Principal Care Management is a Medicare-backed
program that helps patients manage a single chronic
condition that significantly impacts their health. The
program is designed to provide comprehensive care
that’s tailored to the patient’s needs, which can improve
health outcomes and reduce the risk of complications.
Care coordination and ongoing communication are key to
successful Principal Care Management.

When Should You Seek Palliative Care
Support?
It’s never too early to start palliative care. In fact, it occurs
at the same time as all other treatments for your illness and
does not depend on the course of your disease. Palliative
care is available throughout your serious illness. Some
serious illnesses can be short in duration, cured or go into
remission. Palliative care can be Instrumental in coping with
symptoms and stress under these circumstances, as well as
with diseases that may be terminal.
How Is It Paid For?
The billing process for palliative care is similar to a typical
doctor visit. Fees vary depending on the complexity of
your needs. Avow accepts Medicare, private insurance
and personal funds as payment for palliative care. You
may be responsible for deductibles and/or coinsurance as
determined by Medicare or your insurance carrier.
Palliative Care Offers:
• Expert treatment of pain and other symptoms so you can
achieve the best relief possible
• Open discussion about treatment choices, including
treatment for your disease and management of your
symptoms
• Coordination of your care with all of your healthcare
providers
• Emotional support for you and your family

Advance Care Planning
Advance care planning is the process of preparing
for future medical care in the event that an individual
becomes unable to make decisions for themselves. The
goal is to ensure that a person’s wishes are respected
and followed, even if they are incapacitated, while also
providing guidance to family members and healthcare
providers. This proactive approach helps alleviate stress
during difficult times and promotes better decision-making
aligned with the individual’s values and beliefs.
